We are looking to recruit a Health and Well-being Navigator to work with St Martins Homeless Services to support and engage people who use our services to identify, engage and sustain access to a range of health services.
You will work towards improve the health outcomes of people who are using our services by providing support to ensure they maintain ongoing access to the services they need and provide support that is holistic and based on a trauma informed approach that is person centred and flexible.
Within the role, you will actively engage with people who use the service on a one-one basis, providing a comprehensive support package that is sustainable, recovery focussed and goal orientated. You will also carry out street outreach as part of the Pathways Norwich service and support people to access the services they need, as well as assist and facilitate individuals to join, attend and maintain health appointments with their GP and other health providers.
To be successful, a minimum of 2 year’s direct experience of working with service users with at least one of the following support needs: substance misuse, offending history, mental health, learning disabilities, single homeless, and a clear understanding of at least two other related groups, as well as a relevant qualification in the field of housing, social work, or similar profession.
