We are recruiting a forward-thinking individual who is used to delivering person-centred support to vulnerable people, to join our community housing service here at St Martins.
Within the role you will develop positive relationships and provide 1 – 1 support to individuals accommodated there, supporting them in accordance with their support and action plans, in order to promote independent living and move on. You will develop positive relationships with the individual and other agencies, ensuring that every interaction is an opportunity for engagement.
You will be monitoring the progress of the individuals using the homeless outcome star, reducing the risk of homelessness, whilst ensuring that they adhere to their licence agreement and the house rules of their accommodation within the Community.
To be successful you will be confident, determined, and conscientious and have a calm, well organised and flexible approach. Experience of working with people with at least one of the following support needs is required: substance misuse, offending history, mental health, learning disabilities, single homeless, and a clear understanding of at least two other related groups
