We are currently looking for a forward-thinking individual, who is used to delivering person-centred support to vulnerable people, to join the Pathways initiative and help clients with entrenched multiple needs reach their personal goals.
The aim is to provide a ‘no wrong door’ method that will offer a seamless service and build on the strength of the Making Every Adult Matter approach, whilst working directly with people experiencing multiple and complex needs and coordinating services to support their recovery journey.
Within the role, you will have direct responsibility for the assessment and support of individuals using the Pathways Service, where their offending impacts on housing and homelessness and you will need to liaise with all persons identified as appropriate to the support of each individual.
You’ll need recent experience of providing support services to people with multiple and complex needs; knowledge of Norwich based services for the client group is desirable, but not essential. With a calm, well organised and flexible approach, you will be willing to contribute new ideas, methods and processes, in order to improve service delivery. Proficiency using Microsoft Office applications and case management systems is also required.
