Pathways Support Worker – Criminal Justice Liaison

35 hrs a week £19,945 to £23,836 pro rata

We’re looking for a forward-thinking individual who is used to delivering person-centred support to vulnerable people to join the exciting new Pathways initiative and help clients with entrenched multiple needs reach their personal goals.

The aim? To take a ‘no wrong door’ approach that will provide a seamless service and build on the strength of a Making Every Adult Matter approach while working directly with people experiencing multiple and complex needs and coordinating services to support their recovery journey.

This role will have direct responsibility for the assessment and support of individuals of the Pathways Service where their offending impacts on housing and homelessness. Liaise with all persons identified as appropriate to the support of each individual.

You’ll need recent experience of providing support services to people with multiple and complex needs, with knowledge of Norwich based services for the client group useful but not essential. Calm, well organised and flexible in approach, you’re willing to contribute new ideas, methods and processes too, in order to improve service delivery. Proficiency using Microsoft Office applications and case management systems is also required.
