We have a vacancy for a Reablement Support Worker (senior) to work in the community in Norwich to provide specialist support to people with severe and enduring mental health needs, as well as other complex needs such as physical health needs, learning disabilities, and some care needs.
The post holder will work in the community, usually in the individual’s homes, to promote independence and choice. Supporting the individual with the needs identified during their social care act assessment.
To be successful you will be confident, adaptable, and conscientious, have empathy with the people we support and the determination to make a difference.
Experience of working (paid or voluntary) with people with at least one of the following support needs: substance misuse, offending history, mental health, learning or physical disabilities is required.
