St Martins, alongside lead partner Together for Mental Wellbeing, has been selected by Norfolk County Council to belong to a partnership to deliver mental health support to people living across the county. The new service will build on the existing work of St Martins to support vulnerable people in the community. Following a year-long process undertaken by the Council and five CCGs to re-organise local provision, Together was awarded the contract to deliver the service in partnership with Norwich and Central Norfolk Mind, St Martins Housing Trust, West Norfolk Mind and Great Yarmouth and Waveney Mind.
The new model of support has been designed to achieve better outcomes for people by creating more integrated care pathways, meaning someone’s support is more seamless across supported living, community and outreach support as they progress. Support will continue to promote independence, and be responsive to individual need, with community support, outreach support and supported living being provided by a flexible multi-skilled team. A range of recovery-focused support will be on offer, including one-to-one support, peer support, groups and drop-ins.

We are particularly looking forward to working with our new partners to continue supporting people to live as independently as possible in their local community, and to make the most of the community resources available in Norfolk.”
The new service will begin operating in March 2018.
